Program Description
The Population Health Essentials Academy is a 3-day introductory program that provides a comprehensive overview of population health and outlines the fundamentals necessary to improve outcomes and reduce costs.
Population health provides the tools and understanding how to practice population health management, including care coordination, patient engagement, transparency, and accountability for all outcomes. Clinicians and healthcare leaders need to be able to work with and across stakeholder groups to coordinate care across the continuum and to align economic incentives in order to deliver the best possible care at the right time and at the best price.
Through case studies and real-life workforce applications, this program will explore the key components of Population Health and share the essential concepts and components for developing and implementing population health management within your organization.
Population Health Essentials can be combined with the Population Health Management & Strategy Bootcamp.
Program Format
This live online 3-day multimodal educational program will encompass live didactic sessions, daily readings, curated and ad-hoc discussions and interactive live group exercises. Participants will be required to attend all live/on-line sessions.

- Define and describe the population health paradigm and identify models and strategies for population health management and creating a culture of wellness
- Examine the economic concept of risk and apply risk management theory to population health management
- Explain the fundamental role and necessity for data management and analytics for population health, in care delivery, care coordination, patient engagement and organizational decision making
- Discuss the role of quality improvement in the context of population health management.
- Describe ways in which a population health perspective affects healthcare priorities in the areas of transitions of care/care coordination, prevention, public health, and health policy
