Program Description

This 2-day advanced workshop session builds upon concepts learned in Population Health Essentials Academy (July 18-20) and provides the opportunity for participants to focus on developing actionable strategies and practical solutions specific to their organization’s needs. This session will provide professionals already involved in population health initiatives with the skills to develop and implement models of care that improve health outcomes.

Population Health Essentials can be combined with the Population Health Management & Strategy Bootcamp.

  • Advancing Quality and Transformation
  • Value-Based Contracts
  • Data and Analytics
  • Core Competencies for Population Health Management (Pharmacy, Care Management and Case Management)
  • Building and Implementing Successful Clinical Strategies

The Program will feature

  • Live online instructor led sessions featuring interactive discussions, case studies and real-world workplace examples
  • Network with and learn from other participants and healthcare stakeholders facing similar challenges
  • Come away with the skills and knowledge to help focus your organization’s population health strategy
  • Online access to coursework and materials, during and for 6 months after the program

  • Key components of a successful population health management strategy
  • Current quality measure programs and how to implement strategies to improve performance
  • How to develop a strategy to incorporate claims data analysis, promote accurate risk adjustment, and engage clinicians to manage the total cost of care for a population
  • To Identify how models of care coordination and care management can proactively manage patient care across the continuum
  • To Identify opportunities to improve pharmacy and post-acute care utilization and reduce costs
  • Critical success factors for building and executing a data driven clinical strategy

Healthcare professionals, especially clinicians and practitioners with an understanding of population health principles

(Session requirement: successful completion of Population Health Essentials, previous Population Health Academy, or a solid foundational knowledge of Population Health).
